I didn't receive my Share With Doctor report
1) Make sure that you're checking the same email that Cardiogram has saved. To see what email that it is, tap the circle in the top-left of the Timeline pane > tap your name at the top of the sidebar.

2) If you don't have access to the email saved in your Cardiogram profile or that email has a typo, you can change it from that profile screen. Be sure to scroll down and "Save changes" after updating the email.
3) Re-request the PDF after updating your email.
